Abstract
Objectives: A very rare form of congenital heart disease - transposition of the great arteries with neither atrial nor ventricular septal defect - is reported; this condition is incompatible with extrauterine survival. Methods: The anatomy and pathophysiology are evaluated with emphasis on the presence of placenta-dependent systemic oxygenation. Results: Possibilities for rescuing such a patient are discussed and a clinical approach is suggested. Conclusion: Transposition of the great arteries with neither atrial nor ventricular septal defect must be diagnosed in utero and aggressive treatment planned before delivery to allow survival.
